What a BBL is
A BBL combines liposuction (to shape the waist, flanks and back) with fat grafting to the buttocks. Because it uses your own fat, you need enough fat to harvest. Part of the transferred fat does not survive, so surgeons often transfer slightly more than the final volume.
Why safety needs particular attention
BBL has been associated with a rare but potentially fatal complication, fat embolism, when fat enters large veins deep in the buttock muscle. Professional plastic surgery societies have issued safety guidance that fat should be placed only in the layer above the muscle (subcutaneous), using techniques that keep the cannula out of the muscle. Ask your surgeon directly how they inject fat and how they monitor safety.
Who may consider it
Candidates are in good health, at a stable weight, with enough donor fat. People who are very lean may not have enough fat for a noticeable change; buttock implants are an alternative that a surgeon may discuss.
Recovery timeline
- First 2 weeks: avoid sitting or lying directly on the buttocks; use a special cushion or sit on the thighs when sitting is unavoidable. Compression garments are worn for the liposuction areas.
- About 2 weeks: follow-up before flying; the flight needs planning (cushion, walking breaks).
- Weeks 3–8: gradual return to normal sitting and exercise, as advised.
- 3–6 months: the final shape becomes clear after swelling settles and some fat is reabsorbed.
Risks
Beyond fat embolism, risks include infection, fluid collections, fat necrosis (hard lumps), asymmetry, contour irregularities at the liposuction sites, blood clots and loss of volume over time. Risk increases with very large volumes and combined procedures.
Travelling to Thailand for a BBL
Plan about 10–14 days. Ask about clot prevention for the flight, bring a BBL cushion, book an aisle seat and walk regularly. Your surgeon decides when it is safe to fly.
