How voice feminization works
Oestrogen does not change the vocal folds once they have been affected by testosterone, so pitch usually stays low on hormones alone. A feminine-sounding voice depends on pitch, resonance, intonation and speech patterns. Voice therapy works on all of these; surgery changes pitch only.
Voice therapy first
Many people reach a voice they are comfortable with through therapy alone. Where surgery is chosen, therapy before and after helps the new voice sound natural and reduces strain. UCSF guidelines and WPATH SOC-8 describe voice and communication support as part of gender-affirming care.
Surgical options
- Glottoplasty (e.g. Wendler technique): shortens the vibrating part of the vocal folds from inside the throat, with no neck scar; the most commonly used approach today.
- Cricothyroid approximation (CTA): tightens the vocal folds through a small neck incision; the pitch rise can reduce over time.
Surgeons often combine glottoplasty with a tracheal shave when requested.
Recovery
Expect about one week of strict voice rest (no talking), then a gradual return to speaking guided by your surgeon and therapist. The voice may sound hoarse or weak at first and continues to settle over weeks to months.
Risks and limits
Possible outcomes include a smaller pitch rise than hoped, reduced loudness or vocal range, hoarseness, a rough voice quality and, rarely, breathing or swallowing problems. Singing ability can change. Results vary between individuals, so discuss realistic expectations with an experienced laryngologist.
Travelling to Thailand for voice surgery
Plan to stay until your follow-up check after the voice-rest period, commonly around 7–10 days. Bring a notepad or phone app for communicating during voice rest.
