Vaginoplasty in Thailand

Vaginoplasty is gender-affirming genital surgery that creates a vagina, clitoris and labia. Thailand has offered it to international patients for decades. It is a major operation: patients commonly spend about a week in hospital and three to four weeks in Thailand, and regular dilation is needed afterwards to maintain depth.

What vaginoplasty involves

Vaginoplasty creates a vaginal canal, clitoris and labia, usually removing the testes (orchiectomy) at the same time. The aim is genitals that look and function in line with the person's gender, including sensation and, for many, the ability to have penetrative sex. A related option, vulvoplasty (zero-depth vaginoplasty), creates the external genitals without a canal and avoids lifelong dilation.

Techniques

  • Penile inversion: the most established technique; penile skin lines the canal, often with extra skin grafts from the scrotum.
  • Peritoneal vaginoplasty: uses the lining of the abdomen (peritoneum), often robotically assisted, to line part of the canal.
  • Colon (sigmoid) vaginoplasty: uses a segment of bowel; usually reserved for specific situations such as revision or limited skin.

The best technique depends on your anatomy, goals and your surgeon's experience.

Assessment and requirements

Surgeons in Thailand generally follow international guidance such as the WPATH Standards of Care, Version 8. For genital surgery this usually means a documented assessment by a qualified health professional, the capacity to consent, reasonably well-managed physical and mental health, and — unless not clinically appropriate or not desired — a period of hormone therapy (SOC-8 suggests at least six months). Some surgeons still ask for two letters; ask each provider exactly what they require.

Hair removal before surgery

Skin used to line the canal can grow hair after surgery. Many surgeons recommend or require permanent hair removal (electrolysis or laser) of the relevant area beforehand, which can take several months. Ask your surgeon early so you can plan.

Recovery and dilation

  • Hospital (about a week): bed rest at first, a urinary catheter and a vaginal pack or stent; you begin dilation under guidance.
  • Weeks 2–4 in Thailand: follow-up checks, learning your dilation routine and caring for wounds.
  • Months: dilation several times a day at first, becoming less frequent over time, but usually needed long term to maintain depth and width.
  • Return to work: often around 6–8 weeks for desk work; heavy lifting and sex usually wait longer, as your surgeon advises.

Risks

Possible complications include bleeding, infection, wound healing problems, urinary issues (such as spraying or narrowing), loss of depth or narrowing of the canal, changes in sensation, and a rare but serious complication, a rectovaginal fistula (an abnormal connection with the bowel). Some people choose a secondary procedure, for example to refine the labia. Ask each surgeon about their complication and revision rates.

Planning your stay in Thailand

Plan for about three to four weeks in Thailand, with accommodation near the hospital that allows rest and privacy. Many patients bring a companion. Confirm what follow-up is available after you return home and whether your operation notes will be provided in English for your local doctors.

How long do I need to stay in Thailand after vaginoplasty?

Commonly three to four weeks, including about a week in hospital, so your surgeon can check healing and make sure you are confident with dilation before you fly.

Do I need to take hormones before vaginoplasty?

Many surgeons follow WPATH SOC-8, which suggests at least six months of hormone therapy before genital surgery unless it is not clinically appropriate or not desired. Requirements vary by surgeon.

How many letters do I need?

WPATH SOC-8 describes an assessment by a qualified professional; some surgeons in Thailand still ask for one or two letters. Ask each provider directly.

Is dilation lifelong?

Dilation is usually needed long term to maintain depth, although it becomes much less frequent over time. Vulvoplasty avoids dilation because no canal is created.

Do I need hair removal before surgery?

Many surgeons recommend or require permanent hair removal of the skin used in the canal, which can take months, so plan early.

What is the difference between penile inversion and peritoneal vaginoplasty?

Penile inversion lines the canal with penile and often scrotal skin; peritoneal vaginoplasty uses the abdominal lining for part of the canal. Suitability depends on your anatomy and your surgeon.
