What top surgery involves
Top surgery removes breast tissue, tightens or removes skin, and repositions or resizes the nipples to create a flatter, masculine-contoured chest. Liposuction is often used to refine the sides of the chest.
Techniques
- Double incision with free nipple grafts: the most common technique for medium and larger chests. Horizontal scars run along the lower edge of the chest muscle, and the nipples are resized and grafted into position.
- Periareolar: an incision around the areola, for smaller chests with good skin elasticity.
- Keyhole: a small incision under the areola, for very small chests with firm skin.
Smaller-incision techniques leave less scarring but are suitable for fewer people and may have higher revision rates. Your surgeon recommends a technique based on chest size and skin.
Requirements
Many surgeons follow WPATH SOC-8, which does not require hormone therapy before chest masculinization. You will usually need an assessment confirming the decision is well considered and that your health is suitable. Ask each surgeon what documentation they need.
Recovery timeline
- First days: a compression binder and often drains to remove fluid; discomfort is usually well controlled.
- About 1 week: drains are usually removed; nipple grafts are checked.
- Weeks 1–2: follow-up visits before flying.
- Weeks 4–6: gradually return to lifting and exercise on your surgeon's advice; avoid raising arms high or heavy lifting early on.
- Months: scars fade and flatten over a year or more; scar care helps.
Risks
Possible complications include bleeding and haematoma, fluid collection (seroma), infection, partial or complete loss of nipple grafts, changes in nipple and chest sensation, contour irregularities or 'dog ears' at scar ends, prominent scarring and the need for revision.
Travelling to Thailand for top surgery
Plan about 10–14 days so drains can be removed and grafts checked. Pack loose, front-opening shirts, arrange help with luggage and avoid lifting anything heavy. Your surgeon decides when it is safe to fly.
